Our innovative SDL case management programs are designed to help people (18 and older) who have a persistent behavioral health disorder make as complete a recovery as possible while living on their own. Case management services can be provided to your loved one in their own home or apartment or they can rent on a month to month basis a furnished apartment unit from SDL.

SDL case management helps people to live independently despite their behavioral health disorder. SDL provides an effective safety net to prevent relapse and hospitalization. In short SDL empowers greater independence.

SDL case managers assist with finding and keeping a job, managing money, cleaning, shopping and cooking. Concierge services can be provided for housecleaning and bill payment at an extra cost. People can receive SDL case management services for an indefinite period of time.

For case management plus furnished apartment rental the fee ranges from $195 to $300 per day based on the extent of services required and location. For case management alone the fee ranges from $100 to $175 per day depending on the extent of services required and travel time. Fees are subject to change without notice. Insurance generally does not cover these fees.
