The SLS Clinic supports the case management program with

Our innovative SDL case management programs are designed to help adults living with a persistent behavioral health disorder make as complete a recovery as possible.
SDL teaches people how to live independently and provides an effective safety net to prevent relapse and hospitalization.
SDL case managers assist people with finding and keeping a job, managing money, cleaning, shopping, and cooking. They also help their clients keep their appointments with their psychiatrist and therapist.
Rental Case Management provides managed independent living services and a month to month furnished rental apartment. One program fee includes rent and case management services. The fee also covers services provided through the SLS clinic such as group therapy, psychotherapy, medication management visits, and vocational support services.
Case Management only provides similar services but no rental apartment. The program fee is based on the extent of services required and travel time. Psychiatric visits through the clinic are billed separately.
SDL Case Management services are available for an indefinite period of time. Insurance generally does not cover long term or extended care.
